Attendees give consent to Hans Biomed USA, Inc. to contact you for future events and promotions.
VIDEO AND PHOTOGRAPHY
The offline popup is photographed and recorded for various reasons including, but not limited to, education, employee training purposes, video testimonials, and marketing purposes. All popup attendees understand that they may be photographed or videotaped for broadcast or exhibition in any medium, and that Hans Biomed USA, Inc. may use the edited or finished product for any legitimate use. Upon entering Hans Biomed’s popup premises, you grant Hans Biomed USA, Inc. the right to take and use such pictures and/or recordings as well as give Hans Biomed the perpetual right to use your likeness, image, or photograph (collectively herein referred to as “photo”) without compensation or payment of any type. You agree to waive and forever release and discharge Hans Biomed from and against any and all claims or actions arising from the use of your photo.

SECURITY
Attendee is responsible for his/her personal property and assumes entire liability for its losses and damages arising out of attendee’s activities on the popup premises.
ITEMS NOT ALLOWED
Outside drinks, food, and alcohol are not permitted.
CONDUCT
If an attendee is acting in a manner that may cause harm to others in attendance or is otherwise deemed to be disruptive or not following the Hans Biomed USA, Inc.’s instructions or guidelines, the attendee may be asked to leave the popup without being entitled to a refund. By participating in Hans Biomed’s MINT™ popup, you agree to follow the supervision and direction of the Hans Biomed’s staff at all times during the popup.
PROFESSIONAL LICENSING AND STATE BOARD REQUIREMENTS
Hans Biomed assumes no liability regarding state board regulations/requirements for medical, dental, and nursing professionals. Practitioners are solely responsible to understand and follow their medical, dental, or nursing state board requirements. Practitioners are responsible for determining if they are permitted to perform PDO thread procedures in their own practices. Hans Biomed is not responsible for regulations or changes made by professional state boards. Questions pertaining to the practice of medicine, or such other provision of healthcare services, should be directed to your applicable state licensing board.
IMPORTANT DISCLAIMER
Enrollment in the popup does not guarantee that the enrolled licensed healthcare professional is competent to perform the procedure, and the attendee will hold Hans Biomed and affiliated entities and trainers, together with their respective members, directors, managers, officers, employees, agents, and affiliates (collectively, "Indemnities") harmless from any losses, liabilities, and negative results or outcomes whatsoever that a patient may suffer from an attendee’s action or procedure. Furthermore, it is the responsibility of the licensed healthcare professional to understand and adhere to all the laws and guidelines of the state in which they practice medicine or otherwise perform healthcare services. Indemnities may not in any way be held liable for violations committed by an attendee. Hans Biomed’s provision of products, training, or other services to a medical professional, or the listing of a medical professional as using Hans Biomed products or services, including training, does not constitute Hans Biomed’s endorsement, representation of assurance regarding the medical provider’s qualifications, skills or experience, or affiliation. Medical providers are independent of Hans Biomed and subject to their jurisdiction’s licensing requirements. Hans Biomed assumes no liability for services provided by any medical professional.
PRICE
The price of one MINT™ popup ticket is $100 per provider. The popup event provides an opportunity for the attendee to observe MINT™ trainer’s live demonstration on how to use MINT™ PDO threads for the midface and jowls.
REFUND & CANCELLATION
Tickets are non-transferable. A 50% refund will be issued for refund requests received 15 business days or more in advance of the scheduled popup date. However, any refund requests submitted less than 15 business days before the scheduled popup date are ineligible for refunds. Please submit your request in writing to mintinfo@hansbiomed.us, including your full name, payment confirmation number, reason for the refund, and date and city of the scheduled popup event. Refunds will be issued using the original payment method. Please allow up to ten business days to process your refund request. It may be necessary to cancel the popup for reasons beyond reasonable control, such as inclement weather and security events; Hans Biomed reserves the right to cancel or reschedule the event, in which case attendees will be issued a full refund. Additionally, Hans Biomed reserves the right to refund the payment and refuse acceptance of the registration at any time, for any reason. Hans Biomed shall not be responsible for expenses incurred by attendees on non-refundable airline tickets, related travel costs, claims for lost wages, or hotel accommodations in the event the popup is canceled or sold out or refund requests are processed. The popup agenda and instructors are subject to change.
LIABILITY AND INDEMNIFICATION
By attending the popup, you agree to indemnify and will hold Hans Biomed and affiliated entities and trainers, together with their respective members, directors, managers, officers, employees, agents, and affiliates (collectively, "Indemnities") harmless from any claims, damages, losses, liabilities, costs, or expenses arising out of or related to your attendance at the event.
FORCE MAJEURE
No party shall be liable or responsible to the other party, for delay or failure of performance (except for any obligations to make previously owed payments to the other party) caused by or results from acts beyond the impacted party’s reasonable control, including, without limitation, the following force majeure events: acts of God, acts of civil or military authority, embargoes, epidemics, pandemics, government regulation or travel advisories, war, acts of terrorism, riots, insurrections, fires, explosions, earthquakes, hurricanes, floods, loss of power, strikes or lockouts making it illegal, impossible, or commercially impracticable for either party to the hold the event in whole or in part. Once such a condition causing force majeure is removed, each party agrees to use its best efforts to resume the performance of its obligations under this consent form.
CHOICE OF LAW AND JURISDICTION
In the event of any dispute or litigation, the terms and conditions of this registration shall be construed in accordance with the laws of the State of California.
